In-district Enrollment
Students living in the Los Alamos Public Schools district must register at the school site where they will attend school.
To register: print the standardized registration form (PDF) fill it out and bring it to your school site along with a proof-of-residence and and the health forms discussed here. In addition, it is recommended that kindergarten students have a record of a physical examination and a dental examination. The Kindergarten Health Examination by Physician Form (PDF) is available at local Los Alamos pediatricians' offices, the Los Alamos schools, and in the Board Policies and Regulations. The form may be downloaded and taken to the offices of out-of-town physicians. The Dental Health Appraisal Card (PDF) is also available at the schools, local dentists' offices, or on this web site. The emergency medical card contains important information on whom to contact in an emergency. It must be completed each school year and signed by the parent or guardian. It is kept on file at the school. Parents should notify the school if any of the information on the card changes during the course of the year. It is also important to notify the school nurse of any changes in the information on the Health History Form. New Mexico state law requires that all students enrolled in school be immunized against certain diseases, as determined by the New Mexico Department of Health. Parents must present proof of immunization, in English, at the time of enrollment in Los Alamos Public Schools. Immunizations may usually be obtained from a personal physician or Public Health Office. Medical, religious, and conscientious objections may be requested. For information on exemptions, students who are "in process" of completing immunizations, or for help in interpreting the chart of New Mexico School Immunization requirements, please contact a LAPS school nurse. New Mexico currently requires immunizations for diphtheria, tetanus, pertussis, polio, varicella, measles, mumps, rubella, and hepatitis B. Non-immunization carries substantial risk.
